NDIS Appointment

Appointments for NDIS participants can be organized by submitting the Referral form below with your details. Our team will contact you as soon as possible to schedule a time that best suits you. At Fuel Your Health Consultations are available for both self-managed and plan-managed NDIS participants.

Appointments available either:

  • In-person clinic consultations: Ascot Vale Health Group and Mill Park Family Medical Centre.
  • Book a Telehealth Consultation (video consultation where you will receive a link to join via your email).
  • Home Visit (we can also arrange to visit you at home if you are unable to attend our clinics).
